Amerigroup helps you with your Managed Long Term Care benefits

Amerigroup Community Connections is a Managed Long Term Care (MLTC) plan offered by Amerigroup. Amerigroup helps you manage your health care benefits if you:

• are 18 or over
• are eligible for Medicaid 
• have a chronic illness or disability that makes you eligible for services usually provided in a nursing home
• are able to stay safely at home at the time you join the plan

With Amerigroup, you get case management and home and community services. We help answer your questions and guide you through the system.

With Amerigroup, you get benefits like:

• a 24-hour Nurse Advice Hotline to answer health questions
• a Case Manager to help you coordinate your care
