Keep your benefits!

Make sure you renew on time so you can keep getting the
benefits you and your family need. It’s simple!

Every year, the County Welfare Agency (CWA) will send you a form. This form tells you it is time to renew your Medicaid, SSI or NJ FamilyCare benefits.

Be sure to look at the due date on your form. You need to renew your eligibility on time. If your eligibility has ended, you will no longer be enrolled in Amerigroup.


Your state case manager can help you fill out the form. If you have any questions, you can also call or go to the CWA office in your area. These offices are listed in your member handbook.

NJ FamilyCare

There are a number of ways to choose us:

  • Call NJ FamilyCare at 1-800-701-0710 (TTY 1-800-701-0720) to enroll by phone (be sure to listen carefully to the prompts)
  • Call NJ FamilyCare to set up an appointment with a Health Benefits Coordinator at a local community site
  • Fill out the NJ FamilyCare Plan Selection Form that came with your packet and return it in the postage-paid envelope provided


You don’t have to recertify your benefits with the CWA. But you may have to do other paperwork for the Social Security Administration. Go online or call 1-800-772-1213 (TTY 1-800-325-0778) to find out how to keep your SSI benefits.
