1 Card 1 Number 1 Plan

Amerigroup STAR+PLUS MMP (Medicare-Medicaid Plan) provides all your Medicare, Medicaid, and prescription drug benefits. That means you’ll have one plan for all your needs. You only need one ID card for all your services. And, one number to call to reach our Member Services line for questions about all your Medicare and Medicaid benefits.

You’ll have a service coordinator who will make sure all your care and services work together. He or she will work with you to create your plan of care that’s based on your needs and your goals.

We’ll help you take an active role in your care. We’ll put your wants and needs first, which might mean helping you live independently in your home or community.

Respecting you and your choices

Our services are designed to be as helpful to you as possible — and we know sometimes that means putting you in charge. You can choose from the many doctors and pharmacies in our plan. Use our Find A Doctor tool to search for ones near you.

Some members qualify for self-directed services. That means you have more choices and independence. If you receive certain long-term services and supports, you’re able to hire, fire, and supervise your own caregivers.

We understand your relationship with your doctor is important

If your provider isn’t in our plan, you can ask to keep seeing him or her for 90 days after you enroll. If you get long-term services and supports, you can keep your providers for up to 6 months after you join. This will give us time to work with your doctor or help you find a new one so you keep getting the care and services you need.

If you’re already enrolled in a Medicaid plan, it’s easy to switch to Amerigroup STAR+PLUS MMP.

Appointed Representative Information

An appointed representative is a relative, friend, advocate, doctor or other person authorized to act on your behalf in obtaining a grievance, coverage determination or appeal. Those not authorized under state law to act for you will need to sign an Appointment of Representative Form and mail Member Services.
