Need help with making meals and grocery shopping? Getting cleaned up and dressed? Going to your doctor appointments? Call us. As part of your Amerigroup STAR+PLUS MMP benefits, we help you get the home- and community-based services you need to stay in your home with your family and friends.

Get the services you need

Our service coordinators work with you to come up with a care plan. Your care plan says what services you need, when and who will give them to you. Your service coordinator will also check in with you from time to time. He or she will make sure you are still eligible and that you are getting the right amount of services. If you need extra help, you can always call us. Your service coordinator will review your care.

Already get these services?

You’ll get all the same services you qualify for now. Amerigroup STAR+PLUS MMP just makes it easier -- now you can get them through the same health plan that covers your regular doctor visits and prescriptions.

Now, your home and community caregivers are part of your team.

With the Amerigroup STAR+PLUS MMP, you, your primary care provider, and everyone else who takes care of you make up your Interdisciplinary Team (IDT). Your IDT is your care team. You work together to create your care plan and set up the services you need. Together, we all work toward one goal: keeping you healthy.

Need help after hours?

Your service coordinator will always be the first person you should call. But if it’s after business hours and you need help right away, remember:

  • You can talk to a nurse day or night, every day of the week, by calling the 24-Hour Nurse HelpLine at 1-855-878-1784. For Spanish, call 1-855-878-1784. TTY users can call 711.
  • If it’s an emergency, call 911.
